Hospitals and local governments play a critical role in supporting the health and well-being of their local communities. But how do they determine which services, programs, or initiatives are most needed?

Every three years, a Community Health Needs Assessment (CHNA) is conducted to determine the healthcare needs and concerns of the community. When South Monterey County residents participate in these assessments, they contribute directly to shaping the future of healthcare in the region, resulting in better health outcomes for local families.

A Community Health Needs Assessment is utilized by hospitals, public health departments, and other partnering organizations to gather data on their community’s health status and needs and typically includes:

1. Community Surveys where residents and community leaders have an opportunity to share their experiences, priorities, healthcare concerns, and systemic health issues.

2. Data analysis of the results to determine chronic disease rates, social determinants of health, and issues with access to healthcare.

The ultimate goal of a CHNA is to identify potential gaps in services, uncover barriers to healthcare, and develop action plans to address regional health concerns.

Here in Monterey County, the 2022 CHNA revealed, among other issues, a significant prevalence of diabetes, resulting in our hospitals developing resources and programs for diabetes education, prevention and treatment.

As a South Monterey County resident, community participation is not just important, itā€™s essential. Every community member has unique insights and experiences that contribute to a more accurate understanding of local health needs. Your input makes a CHNA truly comprehensive and impactful for all residents. Itā€™s a way to advocate for yourself, your family and our rural community. Itā€™s also an opportunity to ensure that the voices of those often overlooked are included in the conversation.

When you take the survey, youā€™re not just providing feedback; youā€™re directly influencing the types of services and programs that will be prioritized in your neighborhood. Your contribution might lead to the creation of a new initiative, expanded mental health resources, or better nutrition programs for children. Your voice matters and can bring about real change.

Participation in the Monterey County Community Health Needs Assessment is anonymous and takes approximately 20 minutes to complete. It is available in both English and Spanish. We understand that your time is valuable, and we want you to know that your contribution is appreciated.
